Online Donation Reference Sheet
Please have all your supporters clearly write their name and complete mailing address on your pledge sheet to ensure they receive their tax receipt.
Please use one pledge sheet for cheque and cash donations and a separate pledge sheet for online donations. All completed pledge sheets are due no later than June 30, 2025.
